If you require any vaccinations relating to foreign travel you need to complete the travel health questionnaire below, which is also available from reception, detailing when and where you will be travelling and then attend an appointment with our Practice Nurse for your vaccinations.

Travel Health Questionnaire 

It is important that you contact us regarding your travel needs, six to eight weeks (ideally eight weeks) prior to your travel date.  Failure to provide us with adequate notice may result in us not being able to accommodate some or all of your travel vaccination requirements.  In these situations, you will need to make alternative arrangements with an independent travel clinic.  This will result in you incurring a cost from the independent travel clinic.

We ask you to contact us as early as possible as we first need to assess your vaccination needs and then arrange an appointment for you in our Travel Clinic to actually receive the vaccinations.  These vaccines have to be ordered as they are not a stock vaccine.  Your Travel Clinic appointment needs to be at least two weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.
