GP Patient Survey 2018

We are pleased that from the  small sample of surveys this has given results above the national average.  The focus seems to be upon appointments to which we can report that new innovations are being formulated which will include a link on this website to allow an electronic consultation with  a GP replying back by the end of the next working day, plus an ongoing review of our service delivery.  Each month the Friends and Family questionnaire which runs to hundreds of replies  consistently reports more than 90% of patients hold the Health Centre and it's service in the highest feedback category of satisfaction. Our aim remains to continuously improve.

New Patient Registration

We accept new patients who live within the practice catchment area,  please contact reception for further information.  The Holmes Chapel Health Centre is not accepting patient registrations from outside of our catchment area.

You will need to complete a registration form, that you can get from reception.  We also need to have information regarding your ethnic origin as required by the Department of Health and next of kin for our records. You can find these forms in our New Patient Information pack, available from reception.

Alternatively you may print off these forms below, fill them out and bring them in with you on your first visit to the practice.

When you register you will also be asked to fill out a New Patient Health Questionnaire (see below). This is because it can take a considerable time for us to receive your medical records and by providing us with this information we will be able to ensure that you recieve the appropriate medical area as soon as you join the practice.
