Text Updates to confirm consent to use mobile number
We are currently updating our text service and the asking for consent to continue to use the mobile number for appointments, or health updates. Replies can be via the text or ringing into the Health Centre. If choosing to ring us, this can be any time we are open over the next two weeks.
Volunteer Dispensary Drivers needed
You will need to use you own car please. Insurance should be free to add this usage on. We are allowed to pay 45P per mile. If interested please would you drop a note to Dispensary Manager Sarah Taylor, or ring in and leave a message.
Patients may now self refer for a consultation with a choice of providers- see the link under nursing services.
The Health Centre will be closed from 12:30 midday, for staff training, on the following afternoons: Wed 2nd May 2018, Wed 6th June 2018.

Please note that the map below provides an estimation of the Holmes Chapel Health Centre's catchment area. Please contact the Health Centre for confirmation if you live on the edge of our catchment area.
We accept new patients who live within the practice catchment area, please contact reception for further information. The Holmes Chapel Health Centre is not accepting patient registrations from outside of our catchment area.
You will need to complete a registration form, that you can get from reception. We also need to have information regarding your ethnic origin as required by the Department of Health and next of kin for our records. You can find these forms in our New Patient Information pack, available from reception.
Alternatively you may print off these forms below, fill them out and bring them in with you on your first visit to the practice.
When you register you will also be asked to fill out a New Patient Health Questionnaire (see below). This is because it can take a considerable time for us to receive your medical records and by providing us with this information we will be able to ensure that you recieve the appropriate medical area as soon as you join the practice.
